The Navigation Service combines technology and human care to follow up with patients after hospital discharge and promote continuity of care.
THE PROJECT
UNDERSTAND HOW IT WORKS
What are the objectives?
- Prevent new hospitalizations and urgent or emergency care
- Provide guidance on medication use and other health care
- Offer more information, safety and support during recovery
The idea is to offer safe and continuous care, from hospitalization to follow-up at home.
How does it happen?
- After hospital discharge, a health professional contacts the patient by message to schedule the start of follow-up.
- Consultations are conducted via video call
- Messaging channel available at any time to offer support whenever questions arise.
- Follow-up is personalized according to each person's needs and health condition.
- All care is conducted with confidentiality, ethics and respect.
